Timecards

Timecards must be filled out and turned in by 12 NOON on the MONDAY following the week you work. If not turned in by the deadline, your paycheck will be delayed one week. You must fill the timecard out COMPLETELY and have your SUPERVISOR'S SIGNATURE. It is YOUR responsibility to turn in your timecard. A new timecard must be started each Monday or at the beginning of a new assignment during the week. NO ORIGINAL TIMECARD = NO PAYCHECK!

Paychecks

Paychecks can be either mailed on Wednesday mornings or picked up on scheduled days. You must specify which distribution you prefer on the front of this application. Please remember to notify us of any address changes. Once checks are mailed, we relinquish all responsibility for postal delivery. In the event a replacement check must be issued, the employee will incur a stop payment fee of $32.

Ends of Assignments

Ends of Assignments must be reported to BOS. If you fail to notify us, this can void unemployment benefits. If you do not call to be placed on another assignment, we assume that you do not need our service. The available line can be called 24 hours a day, 7 days a week (706-353-3030 option #1).

Absences

Absences must be called into BOS before your shift begins. Please allow sufficient time for us to contact the client. Failure to report an absence or tardiness can result in termination. Acceptance of a job is an oral contract and shows a commitment to BOS. It should take priority over all but emergency situations.

Evaluations

Evaluations may be sent to your supervisor during the assignment or after completion of an assignment. Any response received from the employer will go into your permanent file.

Permanent Placement

Permanent Placement is our customer’s option after you complete 480 hours with their company through our payroll. If a customer wishes to hire you permanently, it is your responsibility to notify BOS.

Injuries

Injuries resulting from on-the-job accidents should be reported immediately to your onsite supervisor as well as to BOS – regardless of the extent of the injury. Delay in notifying BOS will result in denial of payment for medical services rendered. Injuries not requiring emergency medical attention must be seen by a physician on our “Panel of Physicians” in order to validate the claim. The on-site supervisor will determine the need for emergency care. In case of an emergency, you may be taken to the nearest emergency room, but follow-up care must be performed by a physician from the posted “Panel of Physicians” or by a Panel Physician’s referral.


BY SUBMITTING A RÉSUMÉ OR APPLICATION, I AGREE THAT:

I have read the above agreement as well as the following provided documents: (1) Panel of Physicians; (2) “Go Directly to Jail” poster, (3) Substance Abuse Policy Statement; and (4) Safety Bulletin. I understand and agree to abide by all terms and conditions related to the employment agreement. Also, I understand that completion of this application does not guarantee employment and does not in any way obligate BOS. If employment is obtained under this application, I agree to conform to the rules and regulations of BOS. I understand that my employment will be at-will and may be terminated with or without notice at any time by either myself or BOS. I authorize and instruct BOS to make whatever inquiries it deems necessary in connection with my application for employment. I release all such persons from any and all liability resulting from obtaining and using said information. I agree to submit to a test for alcohol or drug use upon request. I hereby certify that the answers given by me to the foregoing questions and statements made are true and correct without reservations of any kind whatsoever and no attempt whatsoever has been made by me to conceal pertinent information. I understand that falsification of any information on this application can lead to immediate discharge at the time of disclosure. I understand my application is active only as long as I maintain weekly contact with BOS.
